By lowering synaptic vesicle uptake, tetrabenazine effectively reduces the transmission of dopamine, which is essential for fine motor movement. This mechanism makes it effective for treating hyperkinetic movement disorders associated with conditions like Huntington's disease.

It's worth noting that tetrabenazine exhibits a low affinity for the dopamine D2 receptor in vitro, indicating that its therapeutic effects are primarily mediated through VMAT2 inhibition rather than direct interaction with dopamine receptors.

Dosage & Administration of Tetrazin 25mg Tablet

For the treatment of chorea associated with Huntington's disease in adults, the recommended oral dosage of tetrabenazine should be individualized and titrated slowly over several weeks to identify a dose that effectively reduces chorea and is well tolerated. The total daily dose can be up to 50 mg/day.

Here are the dosing recommendations:

1. Initial Dose: Start with 12.5 mg taken orally once daily.
2. Week 2: After one week, increase the dose to 12.5 mg taken orally twice daily (every 12 hours).
3. Maintenance Dosing: Slowly titrate the dose by weekly intervals of 12.5 mg/day to identify the lowest effective dose that reduces chorea and is well tolerated.
4. If the daily dose is between 37.5 to 50 mg/day: Administer the dose in divided doses every 8 hours.
5. Elderly Patients: Start with an initial dose of 12.5 mg daily and gradually increase as tolerated.
6. Total Daily Dose >50 mg/day: If a dose higher than 50 mg/day is required, patients should be tested and genotyped to determine if they are poor or extensive metabolizers of CYP2D6. The dose should be individualized accordingly, not exceeding 100 mg/day or 37.5 mg per dose.
7. Moderate to Severe Tardive Dyskinesia: Initially start with 12.5 mg daily and increase gradually based on response.
8. Hepatic Impairment: Tetrabenazine is contraindicated in patients with hepatic impairment, as the dose cannot be adjusted to ensure safe use.

Contraindications
